HIPAA Notice of Privacy Practices

Adopted voluntarily. Massage therapists in Florida are not strictly covered by HIPAA, but we follow HIPAA-aligned practices for intake data.

Protected information

Intake forms capture medical history, medications, prior surgeries, pregnancy status, allergies, and treatment preferences. We treat this information as protected health information regardless of legal requirement.

Access

Your intake record is encrypted at rest and accessible to the therapist assigned to your session and to MT360 administrators. Every administrator access is logged and auditable.

Retention

We retain intake records for the life of your account plus three years, then purge them.

Breach response

We will notify affected users within 72 hours of discovering any unauthorized access.
